Although it is rare for patients to swap from tranylcypromine (Parnate) to phenelzine (Nardil) or vice versa, it must be noted that there must be a full washout (of 4 weeks) because there is a risk of hypertensive crisis and consequent cerebral bleed. The reason for this risk is unknown. It should be assumed that this risk also applies to any other irreversible MAOI, including selegiline. The situation with RIMAs like moclobemide is uncertain.
